We work with you, your family and your care team to provide support that is personalised to your needs and the way you want to live.

Personal Care

Our team can provide respectful assistance with your personal care and daily routines.

This may include:

  • Showering and bathing

  • Dressing and grooming

  • Toileting and continence support

  • Assistance with personal hygiene

  • Assistance getting in and out of bed

  • Mobility and daily routines

  • Support with other personal care needs

We provide personal care in a way that protects your dignity, privacy and independence.

Domestic Assistance

Keeping your home clean and comfortable can become more difficult as we get older.

Our team can assist with everyday household tasks, including:

  • General house cleaning

  • Vacuuming and mopping

  • Dusting

  • Cleaning bathrooms and kitchens

  • Changing bed linen

  • Laundry

  • Washing and putting away dishes

  • Other agreed household tasks

Meal Preparation

Good nutrition is an important part of staying healthy and independent.

We can assist with:

  • Planning meals

  • Preparing meals in your home

  • Preparing meals for later

  • Simple cooking

  • Preparing snacks and drinks

  • Assistance with shopping for food

  • Cleaning up after meal preparation

We can work with your preferences, routines and dietary requirements.

Social & Community Participation

We can provide one-to-one support to help you participate in activities that are meaningful to you.

This might include going for a coffee, visiting a favourite place, attending an activity or simply getting out into the community.

Our support workers take the time to understand your interests and provide encouragement while respecting your choices.

Shopping & Errands

We can assist you with everyday shopping and errands when getting out and about becomes difficult.

This may include:

  • Grocery shopping

  • Picking up prescriptions

  • Collecting household items

  • Shopping for personal needs

  • Other agreed local errands

Where appropriate, we can also support you to attend the shops yourself and remain involved in your own shopping.

Transport & Community Access

Staying connected to your community is an important part of maintaining independence.

Our team can provide transport and support for:

  • Medical appointments

  • Allied health appointments

  • Shopping

  • Hairdresser and personal appointments

  • Community activities

  • Social outings

  • Visiting family and friends

  • Local events and activities

  • Other approved destinations

Where required, we can provide one-to-one support throughout the outing.

Social Support & Companionship

Sometimes having someone there makes all the difference.

Our team can provide companionship and social support in your home or community.

This may include:

  • Having a chat and spending time together

  • Going for a walk

  • Visiting a café

  • Attending community activities

  • Supporting hobbies and interests

  • Visiting friends or family

  • Accompanying you to appointments

  • Helping you stay connected with your community

Medication Support

Where included within your approved care arrangements and provided within the scope of our workers' responsibilities, we can assist with medication-related routines.

This may include support with:

  • Medication prompts

  • Assistance with medication routines

  • Medication-related documentation

  • Liaising with your family or care team where appropriate

Medication support is provided in accordance with your care plan and relevant requirements.

Nursing Services

Our clinical services can provide professional nursing support in the comfort of your own home.

Nursing services may include:

  • Health assessments

  • Wound care

  • Continence support

  • Medication-related support

  • Monitoring of health needs

  • Post-hospital support

  • Chronic condition support

  • Health education

  • Liaison with your GP and other health professionals

  • Clinical advice and care planning

Care Management & Coordination

Managing different services and appointments can sometimes feel overwhelming.

Our care coordination team can help bring your supports together and ensure everyone is working towards the same goals.

Hospital to Home Support

Returning home after a hospital stay can be challenging.

Our Hospital to Home support is designed to help make the transition home smoother and safer.

We can assist with:

  • Preparing for your return home

  • Support after discharge

  • Personal care

  • Domestic assistance

  • Meal preparation

  • Transport

  • Medication routines

  • Shopping and essential errands

  • Community access

  • Liaising with your family and care team

  • Helping establish your daily routine again

We can work alongside hospital discharge teams, families and other professionals to help put the right supports in place.

Respite & Carer Support

We understand that family members and carers also need time to rest and attend to their own commitments.

Our team can provide support to give carers a break while continuing to provide safe and respectful support to the person receiving care.

Respite may be provided in the person's home or through agreed community activities and outings.

Overnight Support

For people who require assistance or reassurance overnight, we can discuss appropriate overnight support options based on individual needs and approved funding.

Support may include assistance with:

  • Overnight routines

  • Personal care

  • Toileting

  • Mobility

  • Reassurance and safety

  • Morning routines

Overnight services are planned according to your individual care requirements.

Allied Health & Other Services

We understand that maintaining your health and independence can involve more than one type of professional support.

Where a service is not provided directly by Inclusion Management, we can work with appropriate external providers and coordinate services such as:

  • Physiotherapy

  • Occupational therapy

  • Podiatry

  • Exercise physiology

  • Speech pathology

  • Dietetics

  • Other specialist services

We can help you and your family connect with the right professionals for your needs.
